Who Are We?
A New Model of Therapy
We are an outpatient clinic that specializes in bringing the outpatient clinic to you. ​​
We provide at-home outpatient physical therapy
for seniors with Medicare Part B.
Visions At Home Physical Therapy is not a home health agency.
Medicare Part B has always allowed outpatient
physical therapy to be provided at home.
To qualify for physical therapy through a home health agency you must be homebound.
We provide outpatient physical therapy at your home, assisted living facility, or community setting.
You do not have to be homebound to receive outpatient physical therapy at home.
You have the right to choose where you receive outpatient physical therapy (At home or the clinic).
We operate like a regular physical therapy clinic, but with one significant difference...We bring therapy to you!

The Visions Balance Program is based on the Otaga Balance Program which is a research-based program that has been proven to decreases fall risk by 40 % in individuals that are at high risk for falling.
The program is administered in the comfort and safety of your home by a licensed physical therapist.
Your therapist will come out 2 times weekly, across 12 weeks to teach you how to properly perform your strength and balance exercises.
You will be expected to perform your exercises independently at home after the initial 12 week clinical phase of the program.
Your therapist will follow up with a monthly phone call or home visit during the self-management phase of the program which last for 4-10 months following the initial 12 week clinical phase.



Louisiana Residents
Louisiana is a direct access state, allowing Medicare Part B and private insurance clients to begin physical therapy without a physician's referral.

Start PT Without Referral

Louisiana Residents have direct access to physical therapy which means you don’t need a doctor’s referral to begin physical therapy treatment.
Some private insurances will not pay for therapy services without a doctor’s referral.
We would be happy to check with your insurance provider to verify whether or not they pay for outpatient physical therapy services without a doctor’s referral.
Direct Access to our therapists means you don't need a doctor's referral.

Medicare Information
We are not a Home Health Agency. Home Health Services are covered by Medicare Part A. We are an Outpatient Clinic. Our services are covered by Medicare Part B. Medicare will not pay for Part A and Part B Services at the same time.
If you currently have a Home Health Nurse or Aide coming to your home we will not be able provide services for you until your Home Health Services have ended.
You do not have to be Homebound to receive outpatient physical therapy services in the privacy of your home. The homebound requirement only applies to Home Health Agencies.
Please let us know if you are currently receiving or have recently received Home Health Services, so we can verify that they have discharged you from their services.
Medicare In-Home Maintenance Program

The Medicare Maintenance Program covers medically necessary skilled physical therapy services to help maintain a patient's current condition or to prevent or slow the decline of the condition.
Under the Medicare Maintenance Program, skilled physical therapy services can be provided for as long as the services are medically necessary to prevent or slow the decline of a patient's condition.
The Maintenance Program does not require that a patient make functional improvements. Skilled physical services can be provided even if no functional improvement is possible.
It is not a restoration program. A restoration program is designed to restore function and requires that functional improvement in a patient's condition be possible.
The maintenance program is designed to help you maintain your current functional level once your potential to improved has peaked.
In-Home Maintenance Therapy

If you have a chronic pain condition that prevents
you from safely performing household or community
activities, weekly in-home pain treatment may allow
you to keep doing the things you love.
If you have a chronic neurological condition
(Stroke, Parkinson, ALS, Muscular Dystrophy),
engaging in the right exercises on a weekly basis
may help you maintain your ability to perform
essential daily activities.
